Prescription drug medications requiring prior approval are noted with a "PA" code on the prescription drug formulary. The formulary can be found here: https://www.aspirushealthplan.com/resources/pharmacy

If your medication requires prior approval, please contact your physician - or you can initiate the prior approval through Optum Rx's site. You will need to create a member account through Aspirus Health Plan (https://aspirushealthplan.com) and once registered, select "View My Pharmacy Information" - which will direct you to Optum Rx's site.

On Optum Rx's site, you can initiate a prior approval request by selecting "Benefits & Claims" and then select "Prior authorization of exception request"
